FORMULRY.ZIP contains the following:

FORMULARY-GDB has been uploaded in almost exactly the same form as I use it on a day to day basis. It is probably the singularly most useful and most used medical application on my palmtop, which I find a very convenient accessory in the practice of medicine.

FORMULARY-GDB is not an extensive analysis of medicines, rather a bare bones database providing useful information, via the very efficient built in database application on the palmtop. It allows for the quick checking of drug dosing and indications for medicines. It has evolved over the last few years to its present state, and it is with that in mind that I post it for general use: as a starting point from which it can be tailored to suit individuals practices/situations. It will likely undergo extensive modification before the end of the year, when my posting changes and I will have access to and need for a very different formulary. This upgraded version will be uploaded at a later date. In the meanwhile, if anybody finds this database useful, and makes any significant additions to it, then please e-mail me and I can incorporate your modifications into the next version.

Latin abbreviations used within database:
ac   = ante cibum (before food)
bd   = bis die (twice daily)
od   = omni die (once daily)
mane = (in the morning)
nocte= (at night)
pc   = post cibum (after food)
prn  = pro re nata (when required)
qds  = quater die sumendus (four times daily)
qqh  = quater quaque hora (every four hours)
(qXh)= (every X hours)
stat = (immediately)
tds  = ter die sumendus (three times daily)

Other abbreviations used within database:
X/60 = X minutes
X/24 = X hours
X/7  = X days
X/52 = X weeks
div doses = in divided doses
im   = intra musclular administration
iv   = intra venous administration
sc   = subcutaneous administration
po   = (per) oral administration
pr   = (per) rectal administration
pv   = (per) vaginal administration

All care has been taken in the writing of this programme, but the author cannot accept any responsibility for any errors in the data, or consequences of using the programme. Please also note that some of the data may be peculiar to the New Zealand medical environment. (Any New Zealanders who happen to download this: I have a selection of other medical programmes and databases that I have written specifically for use in NZ. This programme can be distributed freely, but please do so in its original ZIPPED format containing all four components as listed above.
